This is a preliminary report on a Serious Security Incident (ref: B34-N574LK) which occurred between 2135-02-29 and 2135-03-02 in Lunar Materials Facility 7. This is an initial analysis and may be subject to revision as new information becomes available.

All facts cited here are true to the best of the knowledge and belief of the reporting officer(s).

The purpose of this analysis is not to assign blame to any individual or department for the security failures that led to this breach. Instead, it aims to summarize the known facts and present some initial findings relative to the vulnerabilities that contributed to this incident.

Evaluation of disciplinary actions to be taken against corporate personnel and/or counter-actions against external parties or organizations are not within the remit of this reporting team. These considerations are left to the Department of Human Resources and the Committee on Reprisals respectively. A copy of this report may be attached as an appendix to any recommendations made by these divisions.
